Oml I feel so stupid for not figuring this out sooner but I've been thinking about it and

Because of what Fyodor said to him

Fyodor wouldn't say anything like that without a clear goal in mind and his goal was to convince Nikolai to live to try kill him

Fyodor truly did understand Nikolai that wasn't a lie

And Fyodor told Nikolai exactly what he needed to hear so Nikolai would want to live longer

Obviously Fyodor set that all up for his plan

So the only reason Nikolai lived was because Fyodor told him exactly what he needed to hear to want to keep living
