John knew who the Lady that is Scratch.




First of all, he obviously recognizes her. It's on the tip of his tongue. Like meeting a long lost colleague, and barely remembering they existed. He even asks the painting who she is!! He also keeps on making statements like "it was..." Or "Though she..." Like he's right on the cusp of remembering someone.
Second, you just don't act this way with a stranger. A stranger even a scary one like this doesn't immediately disorient you, and make you focus on her. Even when forgetting about the painting he knows she's important. Even when the importance of the painting isn't in his mind.
Third, John describes her as lovely, in a strange endearing way. Like fantasizing, or recalling a pleasant memory. His descriptions seem loving as well.
Conclusion
I think she was someone of great importance to the KiY, maybe even a relative. Someone that even the KiY would be fond of. And John who only has a vague but terrifying silhouette of his time as king forgot about, but was immediately pulled back into when seeing her. I was thinking a mom or sister, but idk enough about Lovecraft to know.
