Fun fact: This song was targeted by Tipper Gore and the PMRC, who were behind the move to have warning labels placed on albums with lyrics or other content they found disturbing. According to Mick Mars, the song wasn't a call to violence but rather was about "a certain person that we used to work with that we felt we were stabbed in the back by."

Fun Fact: “Headcase” is the second song in October 2023 (after “Bite”) that did have a teaser for a music video before its release date. Consequently, it was the only song from the EP that released without a corresponding music video or teaser post on its official release date. Derivakat eventually revealed the song in the EP’s tracklist announcement post on Twitter/X a day after it released, and the music video the following month.

Brian May supports suggestions that the song contained veiled references to Mercury's personal traumas. He recalls "Freddie was a very complex person: flippant and funny on the surface, but he concealed insecurities and problems in squaring up his life with his childhood. He never explained the lyrics, but I think he put a lot of himself into that song."
