This was an idea that the Twitch chat helped me come up with, it is definitely an ambitious item to try and write a short card for. Using this should be collaborative between the DM and the player because we can see how it could be used in a broken way, but hopefully everyone at the table is on the same page about having fun and not just power gaming it. :P HAVE FUN!
Pen of Possibilities
Wondrous Item, very rare (requires attunement)
“A hefty fountain pen made of silver. It features delicate designs of gold showcasing several scenes from popular fiction. It also features a large pretentious red jewel on the end of it.”
Your Charisma or Intelligence score increases by 2, to a maximum of 20.
Before taking a long rest you can write a single sentence down on a piece of paper predicting a possibility for tomorrow. The sentence can contain no more than 20 words. When you write down the possibility, roll a d100 and on a result of 10 or lower, the possibility will come true. There is always a chance that despite the possibility becoming reality, that it doesn’t happen as you intended it to. The more mundane the possibility the more likely it is to match your intended prediction.
For example finding a small pouch of gold coins on the ground or convincing a shopkeeper to give you a discount, will easily come true. The greater the impact of the possibility, the greater the likelihood that the possibility plays out in an unintended way. Portent Possibility. When you finish a long rest, roll two d20s and record one of the numbers rolled. You can replace any attack roll, saving throw, or ability check made by you or a creature that you can see with this roll. You must choose to do so before the roll.

⚔️ 𝗡𝗲𝘄 𝗶𝘁𝗲𝗺! Tidesplitter Weapon (trident), rare (requires attunement) ___
This sharpened bident is covered in swirling water motifs. You gain a +1 bonus to attack and damage rolls made with this magic weapon. Immediately before you make a melee attack with the weapon while underwater, you can move up to 10 feet without provoking opportunity attacks.
The bident has 7 charges and regains 1d6 + 1 expended charges daily at dawn.
𝙋𝙖𝙧𝙩 𝙒𝙖𝙩𝙚𝙧. While holding the bident, you can use an action to expend 1 or more of its charges to make a dry path through a nearby body of water. Choose a point within 60 feet of you that is within a body of water you can see. The path extends in a direction you choose from that point in a straight line that is up to 120 feet long and up to 20 feet wide. The path also extends vertically, up to 30 feet high. The path remains for 1 minute or until you end the effect using another action, at which point it slowly refills with water over the next 6 seconds. For each charge you expend after the first, you can cause the path to extend 120 feet longer and 30 feet higher, and its duration increases by 1 minute. Alternatively, you can expend 1 of the bident’s charges to use this property to target a creature made of water, such as a water elemental. The target must succeed on a DC 15 Constitution saving throw or take 8d8 force damage as the bident’s magic attempts to split it apart. ___
