Homura is on the right, wears her magical girl outfit (white longsleeved shirt and dark grey skirt) and reaches up towards Madoka who is floating in from the left with her goddess outfit (white bow in her hair and a wide, white dress which layers cascades into a light pink). Their faces aren't depicted but very close to each other. The background shows geometrical shapes in a variety of colors. The colors represent the main five magical girls of the anime series.
Behind the glass painting is a window were sun light is coming through, illuminating the glass painting. Below is a marble windowsill.

I've recently binged through all of Madoka + Rebellion, twice. How much thought has gone into every detail of this show is unbelievable
