Maybe? I don't think he had many coherent thoughts in the moments before he died. Maybe in the sense that he failed to take care of him the way he wanted to.

Leaning into the whole tragic Romeo & Juliet situation of it all


This is all that goes through my head during this scene
While it's rare, there are people who actually survive self-inflicted gun shot wounds to the head (a study I found identified 35 such patients in 10 years at one hospital), so there must also be a few more who survive for a short time but fail to recover, or bleed to death. I simply choose to believe Korn was so destraught and shaky and quick about it, he didn't damage his brain enough to kill him instantly and was still able to have an NDE.
It's not just because I love Korn and am 100% biased (i am), but also, thematically it doesn't make a lot of sense to me for that parallel universe moment to be Tonkla's only, because he did nothing different to change the outcome. We already know Tonkla wanted to be with Korn, what use was there in showing that scene? What would it have communicated if it was only Tonkla's? Just piling on the tragedy?
Remember, Big died for our sins. So we have to sin extra hard every day, to make his sacrifice worth it
