A clinic which receives NHS funding has been promoting prostitution as a way for transgender people to pay for their transition treatment, The Mail on Sunday can reveal.
Being a sex worker ‘can be useful and sometimes empowering’, according to a guide produced by CliniQ, a sexual counselling service for transgender people at King’s College Hospital in London. It adds: ‘It can help us pay for parts of our transition.’
The booklet by CliniQ, which is part-funded by King’s College NHS Trust and three London local authorities, also suggests that transgender men – people born in female bodies but transitioning to male – can hide the fact that they are trans when visiting gay sex parties.

Entitled Cruising: A Trans Guy’s Guide To The Gay Sex Scene, it advises readers to circumvent men-only restrictions at gay saunas by only performing sex acts ‘without others seeing your genitals’. Experts warned doing so would be committing the crime of sex by deception.
The pamphlet, which features crude sexual language including 22 uses of the words ‘f***’ and ‘f******’ in its 44 pages, focuses extensively on extreme sex acts, including sadomasochism and bondage.
‘Sex in public spaces is legal, so long as other members of the public cannot see you,’ it states. ‘Or so long as it is unlikely someone will come across you having sex. For example, having sex in a quiet woodland, away from the road or path, late at night.’
Despite being targeted at pre-operative trans men – many of whom are in their late teens and early 20s – the booklet makes only a fleeting reference to pregnancy.
LGB Alliance, an organisation that stands up for gay and women’s rights, last night demanded that NHS chief executive Amanda Pritchard and Health Secretary Sajid Javid order the booklet to be ‘immediately removed’. King’s College Hospital last night said it had been withdrawn.
In a letter to health chiefs, the alliance said: ‘We are writing to express our alarm that an NHS publication is encouraging the sexual assault of gay men through the criminal act of sex by deception and is promoting prostitution as a way to fund gender reassignment.
‘Young trans men are encouraged to visit gay male saunas and to keep secret the fact that they are not biological males.
‘Not only is this an offence under law, it treats gay men with contempt, and puts in danger the lives of the young trans men who could potentially face a backlash from gay or bisexual men in the saunas.’
Kate Barker, managing director of the LGB Alliance, said: ‘We are astonished that a leaflet encouraging vulnerable girls to undertake criminally deceptive and breathtakingly risky sexual acts in gay men’s saunas and quiet woodlands is being endorsed by the NHS.
‘The fact that this ugly and demeaning guide wraps itself in the rainbow flag is an insult to all the same-sex attracted people who are as horrified as we are by the casual homophobia of CliniQ. Encouraging young women to believe they are really gay men is cruel, dangerous and deeply wrong.’
Tory peer Baroness Nicholson described the booklet as ‘surely outside the professional ethics of NHS medical professionals and may even go beyond the bounds of the law’. She intends to raise the matter in the House of Lords.
CliniQ was the UK’s first dedicated sexual health service for transgender people when it opened at King’s College Hospital in April 2019. It is run in partnership with the hospital and is backed by Dr Michael Brady, a consultant in sexual health and HIV who is also the LGBT national adviser for NHS England.
King’s College Hospital NHS Foundation Trust said: ‘King’s College Hospital was not involved in the content, publication, distribution or promotion of the booklet. It is no longer available on the CliniQ website and it is not being used or distributed at our sexual health centre.’ CliniQ was contacted for comment.
